Part‑time
GENERALThe Resident Care Coordinator is responsible for assisting the Home Services Manager to identify, coordinate, complete, document and market the home services program. The Resident Care Coordinator is responsible for managing the PCA staff schedule to ensure staffing levels are sufficient to provide quality care and services. The Resident Care Coordinator is responsible for assisting PCA staff to find solutions, to understand expectations and to meet those expectations with excellence.
ESSENTIALJOB DUTIES
- Assist with the preparation and management of PCA staff schedules to be approved by the Director to assure consistent and sufficient staffing to meet Residents’ needs.
- Assume on call duties as directed by the Director.
- Provide direct care services to residents within the scope of the Personal Care Attendant job description.
- Assist with the orientation of new PCA staff as directed by the Director.
- Observe Resident/client functioning and notify the Director and/or Health and Wellness Navigator of observed needs and/or concerns.
- Provide care and services according to Resident/client’s home service plan of care.
- Complete appropriate documentation of service provision.
- Communicate the Resident/Client’s response to home service plan of care to the appropriate clinical leader.
- Promote the Home Services Program through educational offerings, marketing/sales visits, other activities.
- Actively participate in appropriate problem‑solving activities keeping the Director informed of any concern that may impact the well‑being of a resident, family understanding, Department policy, and regulatory compliance.
- Encourage a positive relationship between residents, family, and staff by responding to needs in a timely manner and providing information/education as indicated.
- Participate in quality review activities, summarize data, and suggest recommendations for improvement to the Director.
- Keep the Director informed of all significant matters at the time of occurrence.
- Assist with presenting staff in‑service programs.
- Demonstrate initiative in any matter that will promote the improved quality of services to our customers.
- Perform all other duties as assigned.

- Illinois certification as a Nursing Assistant, LPN, or Medical Assistant.
- Current CPR certification.
- A minimum of five (5) years’ experience working with the elderly in a caregiving environment (preferred).
- Previous supervisory and scheduling experience desired; supervisory skills involving good communication.
- Scheduling experience a plus; good organizational abilities.
- Experience leading co‑workers, good problem‑solving and CQI abilities.
- Familiar with established protocols and regulations in order to assist management operations ensuring accuracy and delivery of services.
- Job responsibilities typically require considerable judgment and problem‑solving in the interpretation and analysis of many diverse facts and issues and the application of appropriate policies, guidelines, clinical protocols, and/or precedents. Ingenuity and foresight are often required in the absence of established guidelines or procedures.
