• Administer medications and treatments per physician orders, and document outcomes
• Monitor patient status, perform routine assessments, and report changes to the supervising RN
• Provide ongoing nursing care, including vital signs, wound care, and symptom management
• Assist residents with activities of daily living (ADLs) and promote comfort and dignity
• Maintain accurate charting in the electronic medical record; follow infection control procedures
• Collaborate with the care team to ensure safety, quality of care, and compliance with regulations

• Active Licensed Practical Nurse license in New Hampshire
• Experience in long-term care or geriatric care preferred
• Ability to work all shifts and adapt to a travel assignment
• Strong communication, observation, and critical‑thinking skills
