Job Title:Wound Care Nurse Practitioner (AGNP/CNP) | Mobile Geriatric Care
Compensation:$120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

Schedule:Full-Time | Monday through Friday

- Provide advanced wound care assessments, treatment planning, and ongoing management.
- Conduct routine rounding visits throughout assisted living, memory care, skilled nursing, and long-term care communities.
- Manage medications, diagnostics, treatment plans, and follow-up care.
- Collaborate with physicians, nurses, therapists, caregivers, and facility staff.
- Support chronic disease management, preventative care initiatives, and continuity of care.
- Help reduce avoidable hospitalizations and improve patient outcomes.
- Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).
- Experience in wound care, geriatrics, primary care, post-acute care, or related settings preferred.
- Strong clinical judgment and confidence managing medically complex patient populations.
- Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
