Job Title:Hospice Clinical Resource RN – Admissions, Liaison & Field Support (RN)
Pay:$90,000 – $130,000 DOE plus Full Benefits Package
Schedule:Monday – Friday with on‑call and weekend rotation

- Review referral lists, intake updates, and reports to identify hospice‑appropriate patients.
- Conduct outreach to hospitals, SNFs, and senior living communities for informational visits.
- Partner with intake to prioritize referrals and coordinate admissions and evaluations.
- Complete hospice admissions including eligibility, consents, education, and start of care.
- Provide visit nurse coverage during high census periods or when case managers are unavailable.
- Communicate with discharge planners, social workers, and providers on hospice eligibility.
- Build and maintain strong relationships with referral sources to support continued growth.
- Ensure timely, accurate EMR documentation and adherence to Medicare hospice regulations.
- Step in across the clinical team as needed to ensure seamless patient care delivery.
- Active Minnesota Registered Nurse license.
- Hospice experience preferred or strong clinical background in ICU, oncology, palliative care, or high‑acuity settings.
- Experience with hospice admissions or care transitions strongly preferred.
- Comfortable working independently in a field‑based environment.
- Strong communication, relationship‑building, and clinical judgment skills.
- Highly organized, adaptable, and responsive to changing priorities.
- Reliable vehicle and ability to travel throughout the service area.
