Twin Cities Physicians is seeking an Office Triage LPN to support our providers and more than 4,500 seniors receiving primary care services throughout over 200 senior living communities across Minnesota. This role is ideal for someone who enjoys being the clinical hub behind the scenes, ensuring patients receive timely, coordinated, and compassionate care.
Please Note:This is a non‑patient‑facing, office‑based position
. You will not provide direct bedside care or see patients in person
. Instead, you’ll make a meaningful impact through phone triage, order management, communication, and care coordination

- Monday–Friday schedule with no nights, weekends, or holidays

- Triage incoming phone calls from patients, family members, and senior living staff
- Collaborate with providers and internal support teams to ensure timely care
- Process provider orders, standing orders, and clinical updates
- Review and reconcile medications
- Prepare patient charts and maintain EMR documentation
- Respond to faxes, refill requests, and incoming clinical communications
- Assist with care coordination and ensure timely follow‑up on patient needs
Serve as a key communication link between providers, patients, families, and community staff
- Hold an active Minnesota LPN license
- Have previous experience in primary care, geriatrics, long‑term care, or senior living
- Possess strong communication and telephone triage skills
- Are comfortable spending the majority of the day on the computer and on the phone
- Have excellent attention to detail and organizational skills
- Can prioritize multiple tasks in a fast‑paced environment
- Have experience with EMR systems
; PCC and Eldermark experience are a plus - Enjoy supporting patient care behind the scenes rather than in a direct patient‑facing role
