What You'll Do

• Travel to patients' homes, assisted living communities, hospitals, and skilled nursing facilities to complete hospice admissions.
• Complete comprehensive clinical eligibility assessments and determine hospice appropriateness.
• Educate patients and families about hospice philosophy, services, Medicare benefits, and what to expect .
• Coordinate a seamless transition into hospice services.
• Collaborate with physicians, hospital discharge planners, social workers, intake coordinators, and the interdisciplinary team .
• Complete timely, accurate EMR documentation while ensuring regulatory compliance.

What You'll Bring

• Active Minnesota Registered Nurse (RN) license.
• Experience in hospice, oncology, ICU, palliative care, home health, emergency medicine, or other complex clinical settings.
• Exceptional communication, assessment, and relationship-building skills .
• Confidence discussing hospice care with patients, families, and referral partners.
• Ability to work independently while managing a flexible field schedule.
• Valid driver's license and reliable transportation.
