As a Hospice Clinical Resource RN
, you'll play a critical role in connecting patients, families, hospitals, senior living communities, and providers with compassionate hospice care.

What You'll Do
  • Review referrals, intake updates, and clinical reports to identify hospice-appropriate patients.
  • Partner with hospitals, skilled nursing facilities, senior living communities, physicians, and discharge planners to coordinate hospice evaluations and admissions.
  • Complete hospice admissions, including eligibility assessments, informed consents, patient and family education, and start-of-care documentation.
  • Provide visit nurse and case management support during periods of increased census or staffing needs.
  • Collaborate with physicians, nurse practitioners, social workers, chaplains, hospice aides, and interdisciplinary team members to ensure exceptional patient care.
  • Build and maintain trusted relationships with referral partners throughout the Twin Cities market.
  • Maintain timely, accurate EMR documentation while ensuring compliance with Medicare Conditions of Participation and hospice regulations.
  • Serve as a flexible clinical resource, stepping in wherever needed to support patients, families, and teammates.
What We're Looking For
  • Active Minnesota Registered Nurse (RN) license.
  • Hospice experience preferred, or a strong clinical background in ICU, oncology, palliative care, home health, or other high-acuity settings.
  • Experience with hospice admissions, care transitions, or discharge planning is highly preferred.
  • Strong assessment skills, clinical judgment, and patient education abilities.
  • Outstanding communication and relationship-building skills with patients, families, providers, and referral partners.
  • Highly organized, adaptable, and comfortable working independently in a field-based environment.
  • Reliable transportation and willingness to travel throughout the service area.
  • Competitive salary of $90,000-$130,000 annually.
