Superior Hospice in the Twin Cities area is seeking a Hospice Admissions Nurse (RN) to be the first clinical connection for families entering hospice. Travel to patients' homes, assisted living, hospitals, and SNFs to complete admissions with compassion and clinical expertise.
You will perform eligibility assessments, educate families about hospice philosophy and Medicare benefits, coordinate transitions, and document in EMR while collaborating with physicians and an interdisciplinary team.
