Salary Range: $42.00 - $46.00 Hourly

Pay: $60,000 - $73,000 annually

Optimal Hospice is looking for a Licensed Practical Nurse (LPN) to join their team. As a Licensed Practical Nurse (LPN) at Optimal Hospice, you will play a vital role in delivering high-quality care to our patients. Working closely with our interdisciplinary team, you will provide compassionate support to individuals and their families during their end‑of‑life journey.

Responsibilities
  • Provide direct patient care under the supervision of a registered nurse.
  • Conduct assessments, monitor patient conditions, and communicate findings to the healthcare team.
  • Assist with personal care activities if needed.
  • Offer emotional support and companionship to patients and their families.
  • Collaborate with other members of the interdisciplinary team to develop and implement individualized care plans.
  • Maintain accurate and up‑to‑date documentation of patient care activities.
  • Record clinical/progress notes in the client/patient's clinical record.

Qualifications
  • Preferably one (1) year of experience as a Licensed Practical Nurse with Hospice experience.
  • Has completed a practical (vocational) nursing program and is licensed in good standing in the state of Colorado and furnishes services under the supervision of a qualified Registered Nurse.
  • Must be able to communicate in the language of the client and document client-related activity as appropriate.
  • Possesses adequate and effective written and verbal communication skills.
  • Possesses adequate and effective interpersonal skills.
  • Physical Demands

    While performing the duties of this job, the employee is regularly required to talk and hear. The position is very active and requires standing, walking, bending, kneeling, and stooping all day. The employee must frequently lift or move items weighing 50 pounds.

    Travel

    This position requires daily travel. Mileage is reimbursed.
