Patient Services Coordinator – LPN
The Patient Services Coordinator‑LPN is directly responsible for scheduling visits, communicating with field staff, patients, physicians, and other stakeholders to maintain proper care coordination and continuity of care. The role also assists with day‑to‑day office and staff management.
Responsibilities- Manages schedules for all patients, edits schedule for agents calling in sick to ensure timely patient reassignment, and updates agent unavailability in the worker console.
- Initiates infection control forms as needed and sends the completed “Employee Infection Report” to HR for upload in the worker console.
- Serves as backup during lunch hour and other busy times, receives calls from field staff, assists with weekly case conferences, and refers clinical questions to the Branch Director as necessary.
- Maintains the client hospitalization log, entering coordination notes and sending the electronic log to all office, field, and sales staff.
- Completes requested schedules as tasks appear on the action screen, ensuring staff are scheduled for skilled nurse/injection visits unless an aide supervisory visit is scheduled concurrently.
- Completes requested schedules for all add‑ons and applicable orders:
- Schedules discharge visit / OASIS collection or recert visit following case conference when task appears on action screen.
- Schedules TIF OASIS collection visits and deletes remaining schedule.
- Reschedules declined or missed visits when appropriate.
- Processes reassigned and rescheduled visits.
- Ensures supervisory visits are scheduled.
- Runs all scheduling reports, including Agent Summary Report and Missed Visits Done on Paper Report.
- Prepares weekly agent schedules, reviews them for productivity and geographic issues, and forwards the schedule to the Branch Director for approval before distribution to staff.
- Verifies visit paper notes in the scheduling console as needed.
- Assists with internal transfer of patients between branch offices.
- If clinical, receives lab reports, assesses normality, faxes copies to the doctor, makes a copy for the case manager, and routes to Medical Records Department. Initiates Employee/Patient Infection Reports as necessary.
- If clinical, may be required to perform patient visits and/or participate in on‑call rotation.
- Licensed Professional Nurse (LPN) or Licensed Vocational Nurse (LVN) licensed in the state where you practice.
- At least one year of home health experience.
- Prior packet review / QI experience preferred.
- Certification in coding preferred.
- Valid state driver’s license and automobile liability insurance.
- Excellent communication skills, ability to interact well with a diverse group of individuals, strong organizational skills, and ability to manage and prioritize multiple assignments.
40 hours per week.
Pay Range$48,900 – $66,200 per year. The compensation range may vary based on geographic location and individual qualifications and experience.
