Presbyterian is seeking a skilled Certified Nursing Assistant (CNA) for our outpatient hospice unit. You will play a critical role in assisting patients with their daily activity, ensuring their comfort, and supporting the medical team in delivering high‑quality care.

• Perform specific patient care procedures (such as basic wound care and catheter maintenance) and tasks under the direction of the primary registered nurse or other licensed clinician.
• Complete all procedures, duties, and tasks under the patient's care plan as ordered in conjunction with the physician.
• Provide care in the patient's home, including single‑family residences, assisted living facilities (ALF), or nursing homes.
• Deliver care across skilled home healthcare, palliative, and/or hospice settings.
• Work clinically under the direction of a registered nurse or licensed clinician while independently providing care in the patient’s home setting.
• Plan and navigate routes to patient service addresses based on road/traffic conditions utilizing driving applications when necessary.
• Manage and maintain patient caseload and scheduling to ensure ordered care is delivered timely while taking patient preference into account.
• Monitor, collect, and record information in the patient record.
• Record vital signs, skin, bowel, and bladder function, tolerance to activities, and other patient data as ordered in the Certified Nursing Assistant (CNA) care plan.
• Report any significant problems or changes in patient status to the primary RN/clinician.
• Perform personal care and assist with activities of daily living such as personal hygiene (shaving, mouth care, foot care, peri‑care, and bathing) and emotional support.
• Assist with restorative aide duties for feeding patients and reporting on tolerance.
• Perform basic wound care and catheter maintenance, as ordered and directed by the RN case manager.
• Apply oxygen delivery devices as ordered without altering settings or determining rates.
• Assist patients with portable oxygen, oxygen concentrator, and humidifier, and ensure oxygen safety measures.
• Assist patients with bed and commode transfers, ambulation with or without equipment, including Hoyer lift.
• Assist patients with prescribed exercises including range of motion (ROM).
• Assist patients with taking medications by providing verbal cues/prompts to take oral medications labeled in a container with dose and time, set up by patient, family, or nurse.
• Follow written treatment care plan as established by the team, offering suggestions for modification.
• Actively participate in IDT and Case Conferences.
• Reinforce teaching of patient and family/caregivers, such as personal care techniques, including care of skin, repositioning, and transferring following training by RN or therapist.
• Document clinical care according to agency policy, regulations, and standards.
• Provide simple meal preparation, cleaning of work area, laundry, and respite care as ordered.
• Participate in the agency continuous quality improvement activities as required.
• Perform other functions as required.

• High School diploma or GED
• Certified Nurse Aide-NM
• BLS Certification required at the time of hire.
• 1 year of direct patient care in any clinical setting, in‑home health, or hospice is preferred.
• Good driving record, access to a vehicle, driver's license, and auto insurance.
