- Direct patient care and case management of hospice patients in outpatient settings
- Ensure end‑of‑life care is delivered within quality and utilization guidelines and directed toward achieving desired clinical outcomes for patients and families
- Develop, coordinate and schedule delivery of care through a multidisciplinary team to the patient in the home or community setting
- Assure completeness, timeliness and appropriateness of the patient care plan, its implementation, and ongoing evaluation
- Assist with communication with multidisciplinary team, support services and community resources to ensure plan of care goals are achieved efficiently and cost‑effectively
- Ensure documentation is timely, accurate, complete and reimbursable
- Act as patient advocate
- Precept newly hired nurses and students; serve as a resource and professional role model
- Complete assessments of physical, mental, psychological/social, functional, financial, treatment, resource and spiritual needs of patients
- Plan, organize, direct, coordinate, evaluate and document all aspects of nursing care in accordance with physician/APC plan of care
- Coordinate patient’s care with patient/caregiver, multidisciplinary team and physician/APC
- Intervene consistently with orders and change interventions as needed to meet patient/caregiver needs
- Evaluate effectiveness of interventions and patient response during each visit
- Develop hospice plan of care in conjunction with physician; ensure completion of required assessments per regulations and QAPI measures
- Coordinate implementation of plan of care through home care team and community resources; ensure compliance with regulatory requirements
- Maintain department productivity standards for visits and caseload management
- Self‑schedule patient caseload and update schedule throughout the day as needed
- Inform patients of visit timing, unplanned schedule changes and accommodate preferred schedule when available
- Maintain communication with multidisciplinary team and obtain orders for care plan changes; document thoroughly in home
- Delegate plan of care to appropriate LPNs and CNAs
- Participate in agency continuous quality improvement (QAPI) and performance improvement project (PIP) activities as required
- Assume other leadership roles within the department as required
- Provide holiday and selective weekend/evening coverage call for agency and patient home visits on a rotating basis
- Perform other functions as required
- Associate Degree in Nursing from a State of New Mexico or Compact State Nursing License
- BLS certification required at time of hire
- One year in direct patient care setting or home health care
- Hospice experience preferred
- Current driver’s license and auto insurance
