• Provides resident care in accordance with standards of practice and within the scope of LPN license.
• Observes and reports on residents' conditions/changes and documents in accordance with facility policy and as required by DHH regulations.
• Administers medication and treatment as prescribed.
• Receives, transcribes, and executes physician orders and follows all 3rdparty instructions.
• Implements and evaluates resident's plan of care upon intake, status change and every ninety (90) days.
• Invent
  01Y and ordering of equipment and supplies; notifies supervisor when supplies are needed.
• Ensures appropriate and timely response to resident and/or family concerns.
• Works closely with the Director of Nursing and Medical Director as needed.
• Informs CNA/PCA staff about the condition of residents and the expectations and needs for the shift.

• Display dependability by being punctual and maintaining consistent attendance.
• Possess basic computer and electronic ability in order to complete the required onboarding, training and job-specific tasks.
• Ability to work closely and efficiently with other CNAs/PCAs and all Francois Bend staff.
• Clear communication and teamwork with all departments to provide well-rounded care.

• Must have a current, valid LPN license.
• Education and/or experience in long-term care, acute care or geriatric nursing preferred.
• Must possess the ability to follow verbal and written instructions, correspondence, and memos.
• Must have reliable transportation.
• Must be CPR and First Aid Certified.
• Must be able to successfully pass a background check
