Position Overview

The RN Case Manager is responsible for managing and delivering the full plan of care for each home health patient assigned, continuously evaluating personal and professional performance and making necessary changes to increase productivity and improve the quality of care delivered.

Responsibilities
  • Create, implement, execute, and coordinate the overall plan of care to promote wellness and prevent re-hospitalization for patients in their homes.
  • Plan and exchange information with physician, agency personnel, patient, family, and community resources to maintain continuity of patient care.
  • Conduct initial and/or comprehensive nursing evaluation visits, determine primary focus of care, develop the plan of care within state-specific guidelines, and ensure patients meet home health eligibility and medical necessity guidelines as defined by the payer.
  • Maintain and submit accurate documentation and clinical notes, including the patient’s condition, appropriate goals, interventions, and patient’s response to home health care.
  • Establish and maintain an effective and efficient visit route.
  • Provide high quality clinical services within the scope of practice as defined by the state laws governing nursing practice, in accordance with the plan of care, from admission through discharge.
Qualifications
  • One (1) year of clinical RN experience.
  • Current RN license in the state where services are rendered.
  • Current CPR certification.
  • Driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation; ability to reliably commute to multiple patient residences daily within the geographic area assigned.
