** Job Description Summary
** Field Nurse Practitioner to provide compassionate, high-quality care to patients across various community-based settings. This is not a remote position — you’ll deliver in-person care throughout your assigned area, Maricopa County, traveling to patient locations as needed.

* * Provide comprehensive primary and acute care to patients in Assisted Living, Memory Care, Hospice, and Group Home settings in Maricopa County.
* Order, interpret, and evaluate diagnostic tests to assess and manage patients’ health conditions.
* Deliver health education, counseling, and disease prevention guidance to promote wellness.
* Collaborate with physicians, nurse practitioners, and other healthcare professionals as part of an interdisciplinary care team.
* Develop, document, and update individualized care plans.
* Prescribe medications and other appropriate treatments in accordance with licensure and scope of practice.
** Requirements:
*** Requires an MS in Nursing
* Minimum of 1 year of related experience with clinical emphasis for nurse practitioner; or any combination of education and experience, which would provide an equivalent background.
* Current unrestricted RN license and NP license in applicable state required.
* Satisfactory completion of a Tuberculosis test is a requirement for this position.
*** This position is bonus eligible based on individual and company performance.
