Position SummaryRegistered Nurse, Wound Care Per Diem – responsible for 4–6 hours per week providing wound care services and consultation across departments.
Key Responsibilities- Work 4–6 hours per week providing consultation to departments for complex wound and ostomy patients according to Wound Care Nurse standards.
- Collect patient data, complete required forms, and respond appropriately.
- Identify patient problems/needs, set priorities, and develop individualized care plans.
- Assess patients with rashes and radiation burns; evaluate comprehension of care plans.
- Educate patients and caregivers, documenting education in EHR.
- Provide basic wound care training and orientation to new nursing staff; deliver ongoing in‑services and training related to wound care and ostomies.
- Maintain and care for patients with ostomy/ileostomy and urostomy.
- Care for skin grafts and various wound types: pressure ulcers, diabetic ulcers, venous/arterial, surgical, burns (including radiation dermatitis, trauma, palliative, dermatologic), skin tears, irritations, breakdown from incontinence, etc.
- Access wounds, create plan of care, set goals, frequency/duration of treatment and projected outcomes.
- Provide treatments/procedures and other care as prescribed, adhering to accepted practice standards.
- Explain nursing procedures, provide discharge teaching, identify emergency situations, initiate advanced therapy, advocate for patients, coordinate care, and evaluate patient care measures.
- Show respect for patient rights and manage ethical issues in patient care.
- Minimum 2 years of wound care experience.
- Bachelor of Science in Nursing (BSN).
- Nationally recognized wound certification (e.g., WCC or WOCN).
- American Heart Association Basic Life Support (BLS).
- Demonstrated ability to provide advanced wound care, including skin grafts and various wound types.
