- Provide discharge/aftercare planning referral services for successful community reintegration (employment, vocational rehabilitation, housing, etc).
- Assists other peer supports.
- Assist with implementation of treatment plans in collaboration with client to reach desired treatment outcomes and intervention strategies.
- Complete assigned monthly reports.
- Participate in multi-disciplinary treatment team for purposes of program evaluation development & case management consultation.
- Maintain clinical documentation on all clients & all services provided from intake to discharge.
- Ensure clients’ confidentiality & clients' rights are maintained.
- Participate in staff meetings and attend training as required.

Must successfully complete four (4) weeks of SCDC Basic Training upon hire.
Minimum And Additional RequirementsState of South Carolina
Minimum Requirements:
A high school diploma and relevant human services or social services experience. A bachelor's degree may be substituted for the required experience.

Minimum Requirements:
A high school diploma or GED and relevant human services or social services experience. A bachelor's degree may be substituted for the required experience.
Must be credentialed as a certified peer support specialist (CPSS) through Addiction Professionals of South Carolina (APSC).
