The Day Services Coach provides psycho-educational day treatment groups, activities and interventions according to each members’ plan of care. This typically occurs M-F during the day assigned to their team with a Recovery-oriented, person-centered approach. The Day Services Coach works within a Treatment Team and under the supervision of a Mental Health Professional and Program Supervisor to provide clinically appropriate activities while assuring that all members are engaged and active throughout the day.
QUALIFICATIONS- 1. High School Diploma/GED is required but prefer bachelor’s degree or equivalent experience resulting in broad knowledge of adult day rehab programing, supervising, training, direct service delivery, budget management, leadership, communication, and time management.
- 2. Must possess a valid and current driver’s license.
- 3. Certified as a QBHP.
- 4. Prefer knowledge of the mental health system, Medicaid rules, CSSP, CARF guidelines related to adult day rehab services.
- 5. Must have proficient computer skills and basic knowledge of word and excel programs.
- 6. Honest, dependable, self-motivated, and dedicated to Birch Tree Values and Mission.
- 7. Ability to perform essential duties and responsibilities satisfactorily.
- 1. Must work under the direction and supervision of a Mental Health Professional.
- 2. Attend weekly supervisions and monthly observed interventions with no more than 13 days between.
- 3. Late services cannot exceed 8% with a goal of 0 – 4%.
- 4. Must complete QBHP certification and complete mandated 8 hours of continuing education training once per 12-month period to maintain QBHP certification.
- 5. Must complete Face to Face MHP Supervision every 14 days with a minimum of 12 contacts every 90 days and Monthly Observed Interventions.
- 6. Communicate daily with MHP’s regarding any concerns/progress as indicated in verbal feedback from MHP, evidence of contribution to the end of shift email summary, and supervision documentation.
- 7. Must be able to do housekeeping duties, such as, but not limited to, laundry, sweeping, mopping, cooking meals, kitchen clean up, bathroom cleaning, etc in order to maintain a clean and safe treatment environment and residential environment as shown by passed quarterly inspections quarterly.
- 8. Ability to read and understand the members’ plan of care to effectively provide services based on their goals and assist in maintaining a safe, clean, healthy treatment and residential environment.
- 9. Must be able to work shifts assigned based on the needs of the members and/or branch. Every effort will be made to maintain a consistent schedule but there may be times that schedules change on short notice.
- 10. Follow program schedule and facilitate psycho-educational groups in consultation with Therapists and/or Program Director and Assistant Director.
- 11. Understand and effectively use program materials, ensuring supplies readily available to conduct scheduled groups.
- 12. Assure that all members assigned to their team are engaged in treatment services, have access to appropriate community resources, and get to appointments on time.
- 13. Must complete all required documentation by the end of their shift every day and ensure it meets standards showing that the LSC is providing services based on the members POC.
