Nurse Practitioner or Physician Assistant, Per Diem

• Provide in-home health evaluations in members' residences
• Conduct approximately 45-minute visits
• Review medications and medical history
• Perform physical evaluations
• Perform additional diagnostic tests when ordered by the member's health plan, including diabetic eye exams, spirometry, and bone density screening
• Use an iPad and clinical workflow for each evaluation
• Build trusted relationships and help improve health outcomes
• Conduct virtual visits as needed
• Provide Signify Health services, including Diagnostic & Preventive Services
• Travel locally and within licensed states as needed
• Help connect members to appropriate care

• Active, unrestricted license(s) in coverage area(s)
• Board certification required
• Multi-state licenses preferred
• Nurse Practitioner or Physician Assistant qualification
• Able to cover Penobscot and surrounding area
• Able to travel locally and within licensed states as needed
• Able to safely perform duties in members' homes, including environments with household pets, secondhand smoke, and airborne allergens or irritants

Demonstrates expertise in conducting in-home health evaluations, performing physical assessments, and utilizing clinical workflows to enhance patient care. Holds active nursing qualifications and board certifications, ensuring compliance with health standards and effective patient management.
