WEEKEND REGISTERED NURSE RN - HOME HEALTH

$7,500 Sign-On Bonus

This Weekend RN opportunity offers a consistent schedule with flexibility built in.

SCHEDULE

* 12-hour shifts

* Saturday and Sunday required

* One weekday of your choice

* Full-Time status

LOCATION

* St. Joseph and Elkhart Counties

* Must be willing to travel within coverage area

As a Home Health RN with Elara Caring, you provide one-on-one care in the patient's home. You manage your own caseload, build meaningful relationships, and see real progress in real time.

- WHAT YOU WILL DO

* Manage and coordinate your own home health caseload

* Complete Start of Care, follow-up, and discharge visits

* Perform comprehensive patient assessments

* Develop and update individualized plans of care

* Complete OASIS documentation accurately and timely

* Educate patients and families on medications and disease management

* Monitor patient progress and report changes to physicians

* Collaborate with interdisciplinary care team members

* Ensure documentation meets compliance and regulatory standards

- WHAT YOU NEED

* Active, unrestricted RN license

* At least 1 year of nursing experience

* Willingness to travel within the coverage area

* Reliable vehicle, valid driver's license, and auto insurance

* Home Health experience preferred

* Sit, stand, bend, lift and move intermittently and be able to lift 50-100 lbs.
