Alpha Omega Hospice is seeking a Full Time Hospice RN Case Manager to join our nursing team providing exceptional end-of-life care in Mineral Wells and Weatherford, Texas. Schedule is Monday–Friday, 8 am–4:30 pm.
You will deliver hands-on hospice care, coordinate services, and support patients and families on their hospice journey. The ideal candidate holds an active RN license, at least one year of RN experience in Med-Surg/acute care (hospice a plus), and is proficient with EMR systems.
