Overview

- Schedule: Full-Time - 3pm - 3:30am - Monday thru Friday & every 3rd weekend

- Use independent professional judgment, analytical skills, and the nursing process to provide a full range of delegated, interdependent, and independent nursing services to patients.
- Within the framework of the Seven Dimensions of Patient Care and Benner's Domains of Nursing Practice, demonstrate clinical competence; compassion and customer service orientation; focus on process and outcomes; and cost-consciousness when assessing, planning, implementing, and evaluating nursing care provided to patients.
- Provide patient care that reflects respect for patient rights, dignity, values, culture, preferences, and expressed needs. Assess patient/family needs for information and education, plan and implement patient teaching, and evaluate effectiveness of educational interventions. Involve patient, family, and significant others in the development of the care plan.
- Collaborate with patient/family, hospital staff, and community agencies to develop discharge plans for continued care needs.
- Orientation Self-Evaluation and performance standards reference the Seven Domains of Nursing Practice and related elements of Signature Care in daily interactions with patients.
- Complete assessments for risk factors (e.g., fall, skin breakdown, VTE, aspiration); initiate appropriate skin/fall prevention measures and medication reconciliation; establish and document the plan of nursing care with patient/family.
- Initiate problems and outcomes list (plan-of-care) based on medical diagnosis and patient needs; monitor, document, and report patient response and progress toward outcomes.
- Document nursing interventions (NIC) and progress toward outcomes (NOC); provide patient education and evaluate effectiveness each shift as required.
- Educate patients regarding treatment plans, safety measures, medications, and self-care; include education in discharge planning.
- Review discharge plans/instructions with the patient prior to discharge and ensure appropriate follow-up.
- Ensure core measures discharge instructions are provided to applicable patients.
- Document patient care according to established guidelines; administer therapeutic regimens and medications per policies and procedures.
- Establish peripheral IV therapy, obtain specimens, and perform or assist with procedures according to standards of care.
- Monitor patient responses, vital signs, labs, and I&O; respond to changes and communicate with the team and physician as needed.
- Recognize and respond to acute changes in respiratory, neurologic, cardiac status, or urinary output; initiate appropriate interventions and notify physician/SWAT when needed.
- Respond to life-threatening emergencies with CPR and the rapid assembly of emergency resources.
- Promote a safe environment, adhere to universal precautions, and manage resources cost-effectively; participate in quality improvement activities and mentoring of student nurses.
- Support organizational and unit goals; contribute to team communication and patient care assignments; assist in workload management and change-of-shift handoffs using the 4 Ps (patient, pertinent issues, picture, plan).
- Maintain professional communication with patients, families, and colleagues; resolve concerns promptly and document appropriately.
- Maintain confidentiality and ethical standards; provide care that respects patient rights and preferences.
- Demonstrate ability to walk/stand for extended periods and to assist with patient mobility as required.
- Licensed Registered Nurse credentialed from the Michigan Board of Nursing obtained within 60 days of hire or transfer date.
- Certified BLS Provider specializing in Basic Life Support from the American Heart Association obtained within 90 days of hire or transfer date, or Certified Instructor.
- Graduate of an approved professional (RN) nursing program; BSN preferred.
- Minimum one year of nursing experience (ER preferred).
