GENERAL SUMMARY
  • Use independent professional judgment, analytical skills, and the nursing process to provide a full range of delegated, interdependent, and independent nursing services to patients.
  • Within the framework of the Seven Dimensions of Patient Care and Benner's Domains of Nursing Practice, demonstrates clinical competence; compassion and customer service orientation; focus on process and outcomes; and cost-consciousness when assessing, planning, implementing, and evaluating nursing care provided to patients.
  • Provide patient care that reflects a respect for patient's rights, dignity, values, culture, preferences, and expressed needs. Assesses patient/family needs for information and education across the continuum, plans and implements patient teaching using a variety of techniques and methods, and evaluates effectiveness of educational interventions.
  • Collaborate with patient/family, hospital staff, and community agencies to develop discharge plans that prepare patients for continued care needs.
  • Standards of Performance across Seven Domains of Nursing Practice, including incorporating elements of Signature Care in daily interactions, being present and engaging, and executing routine nursing assessments.
  • Perform comprehensive assessments for risk factors including fall, skin breakdown, VTE, and aspiration. Initiate appropriate initiatives and complete medication reconciliation upon admission and change in level of care.
  • Initiate Problems and Outcomes list (plan-of-care) based on medical diagnosis and patient needs, monitor, document, and report patient response to interventions and progress toward outcomes.
  • Educate patient regarding treatment plan, safety measures, medications, and self-care as indicated. Work with Case Manager to ensure appropriate referrals before discharge.
  • Review discharge plans/instructions with patient prior to discharge and ensure appropriate follow-up is arranged.
  • Ensure core discharge instructions are given to all CHF patients.
  • Document patient care according to established documentation guidelines.
  • Demonstrate knowledge and skill application of basic nursing procedures: dressing change, catheterization, NG tube insertion, suctioning; implement medical interventions; maintain peripheral IV therapy; obtain body fluid specimens; perform blood capillary glucose monitoring.
  • Administer medications, IVs, blood products according to established policies.
  • Prepare patients for surgery or other invasive procedures per guidelines and assist with procedures per standards of care.
  • Utilize and troubleshoot equipment such as IV pumps, PCA pumps, feeding pumps, and patient beds.
  • Monitor patient responses, review work in progress, ensure timely orders, and communicate changes using SBAR or other standardized approaches.
  • Recognize acute changes in respiratory, neurologic, cardiac, and urinary status and respond appropriately, including initiating emergency protocols such as CPR.
  • Support families in crisis and provide resources for grieving processes.
  • Communicate with other professionals, maintain safety of environment, and use restraints per policy.
  • Adhere to universal precautions and cost‑effective use of supplies.
  • Support nursing department and unit goals, participate in quality monitoring and improvement activities, and mentor student nurses.
  • Evaluate own performance, utilize peer and manager feedback for continual improvement.
  • Complete yearly safety and unit specific education requirements (GLI).
  • Collaborate on patient care assignments, communicate expectations, observe patients, and assist team members as needed.
  • Use the 4 Ps (patient, pertinent issues, picture, plan) for shift change communication.
  • Maintain a healthy work environment through respectful communication and conflict resolution.
  • Respond to unit workload changes and staffing levels.
  • Possess interpersonal skills necessary to interact with patients, families, staff, and physicians.
