CASE MANAGER

Altru Rehabilitation Hospital, part of Altru Health System in Grand Forks, ND, is seeking a compassionate Case Manager to coordinate care for inpatient and outpatient rehab patients. In this role, you will manage a caseload from admission through discharge, conduct comprehensive assessments, develop individualized care plans, and collaborate with an interdisciplinary team to support functional recovery and independence. You will educate patients and families, arrange post-acute services, and connect patients to community resources.

Responsibilities

• Manage a caseload of rehab patients from admission through discharge
• Develop and coordinate individualized care plans with the interdisciplinary team
• Conduct assessments of patient needs, progress, and barriers to recovery
• Educate patients and families on treatment goals, resources, and discharge plans
• Coordinate post-acute services, referrals, and community resources
• Advocate for patient needs and ensure a patient-centered care experience
• Document assessments, care plans, and progress notes accurately and timely
• Collaborate closely with physicians, therapists, and nursing staff
• Participate in care conferences and quality improvement initiatives
• Support continuous learning and adherence to hospital and regulatory standards
