On-call requirement: minimum one weekend per month
Position Purpose
The Clinical Care Coordinator plays a key role in supporting clinical operations and ensuring residents receive safe, respectful, and high-quality care in an assisted living environment. This position works closely with the Resident Services Coordinator and interdisciplinary care team to support medication oversight, staff training, regulatory compliance, and resident well-being while promoting dignity and independence.
Key Responsibilities
Resident Care and Clinical Oversight
Ensure resident care and service plans are implemented accurately and respectfully
Treat all residents with dignity and respect at all times
Review new physician orders daily and ensure complete processing
Monitor lab orders, follow up on results, and communicate as appropriate
Review monthly summaries, vital signs, and resident weights
Communicate with licensed healthcare professionals regarding resident condition, medications, and treatments
Medication Management
Monitor Coumadin logs and medication documentation
Spot-check Medication Technician charting per episodic charting logs
Conduct medication pass reviews twice per year with all Med Techs
Investigate medication errors and forward required documentation
Complete monthly medication cart audits
Review pharmacy delivery logs and physician notifications prior to faxing
Ensure resident medications are available and administered properly
Wound Care and Preventive Services
Review pressure ulcers weekly and ensure accurate measurement and documentation
Complete weekly PUSH tools and verify care plan documentation
Track and administer chest X-rays, TB skin tests, TB questionnaires, and B-12 injections
Staff Training and Support
Assist with Med Tech training for new hires and promotions
Assist with training Resident Services Assistants and Shift Supervisors as directed
Complete Med Tech performance reviews (90-day and annual)
Monitor Licensed Health Care Professional logs and coordinate resident visits
Compliance and Operations
Uphold resident rights, confidentiality, and HIPAA standards
Enforce infection control protocols
Ensure compliance with Homes for the Aged regulations and Employee Handbook guidelines
Check medication rooms and refrigerators for cleanliness and expired medications
Participate in Manager-on-Duty rotation, safety meetings, and infection prevention meetings
Perform on-call duties as required
General Responsibilities
Serve as a positive ambassador for the living center
Comply with Life Safety policies and emergency drills
Complete required documentation accurately and timely
Participate in required training opportunities
Report safety or health hazards to the Resident Services Coordinator
Work cooperatively across shifts and departments
Respond to resident requests for assistance, including call lights
Perform other duties as assigned to support living center operations
Qualifications
Licensed Practical Nurse (LPN) in the State of Michigan preferred
Proof of TB testing training
Proof of medication administration competency
CPR certification required
Strong verbal and written communication skills in English
Ability to supervise and support care staff
Compassionate and professional approach to resident care
Ability to pass state-mandated criminal background screening and random drug testing
Physical and Mental Requirements
Ability to lift up to 50 pounds
Ability to stand, walk, bend, stoop, kneel, and assist with resident personal care
Ability to communicate effectively with residents, coworkers, and supervisors
Emotional stability to support residents with chronic illness, dementia-related behaviors, and end-of-life care
