Hospice Registered Nurse; RN Case Manager

Position Overview — Sign-On Bonus $10,000

As a Hospice RN Case Manager at Trinity Health At Home, you are fully responsible for the client assessment, care planning, therapeutic intervention, and overall supervision of client care and outcomes. You serve as the designated case manager for hospice patients, collaborating with physicians and other hospice team members to anticipate and manage patients’ symptoms, assist in identifying patient, family, and caregiver grief/loss issues, and implement the hospice plan of care.

• Active RN license in Michigan
• 1+ years of clinical nursing experience (Hospice preferred)
• Strong communication, assessment, and organizational skills
• Compassionate, dependable, and mission‑minded
• Valid driver’s license and reliable transportation required
