Job Responsibilities
  • Assist the Social Services Director in implementing and carrying out social service programs in accordance with state and federal guidelines
  • Support the identification and documentation of residents' social, emotional, and psychological needs
  • Provide service coordination and support for residents and families, including communication and follow-up
  • Participate in care planning and interdisciplinary team meetings as assigned
  • Maintain accurate, timely documentation to support compliance and regulatory standards
Qualifications
  • Associate's Degree in a Human Services, Social Work, Psychology, or related field preferred, but not required (experience may be considered in lieu of degree)
  • Experience working in a skilled nursing or long-term care setting preferred
  • Strong written and verbal communication skills, with the ability to interact professionally with residents, families, and interdisciplinary teams
  • Ability to assist with documentation, assessments, care planning, and regulatory requirements under the direction of a licensed Social Services Director
  • Compassionate, resident-focused approach with strong interpersonal skills
  • Organized and detail-oriented, with the ability to manage multiple priorities
  • Working knowledge of resident rights, discharge planning, and psychosocial support services preferred
  • Positive, can-do attitude with excellent attendance
