As an RN Case Manager, you'll have the opportunity to provide individualized care while serving as a trusted advocate for your patients. You'll manage and coordinate care plans, collaborate with physicians and interdisciplinary team members, and guide patients through their home health or hospice journey with compassion and clinical expertise.

• Current Registered Nurse (RN) license
• Current CPR certification
• Strong clinical assessment and critical thinking skills
• Ability to independently manage patient care and caseloads
• Excellent communication and care coordination abilities
