We are seeking a Full-Time Licensed Practical Nurse (LPN) to join our Home Health & Hospice team serving Grand Island and surrounding communities. This rewarding position allows you to provide personalized nursing care while working closely with patients, families, physicians, and an interdisciplinary care team dedicated to improving quality of life.
What You'll DoAs a member of our Home Health & Hospice team, you will:
- Deliver skilled nursing care in patients' homes
- Monitor patient conditions and communicate updates to the healthcare team
- Assist with implementing individualized care plans
- Educate patients and families on health conditions and care needs
- Provide comfort-focused care and support to hospice patients and their loved ones
- Promote patient independence, safety, and overall well-being
Qualified candidates will have:
- Current, active Nebraska Licensed Practical Nurse (LPN) license
- Current CPR certification
- Strong clinical assessment and communication skills
- Compassionate, patient-centered approach to care
- Excellent organizational and time-management abilities
- Ability to work independently while collaborating effectively with a team
- Commitment to providing exceptional service and care
