Location: Grand Junction, Colorado
Schedule: Flexible Part-Time or PRN (Weekday Hours)
Pay: Competitive hourly rate based on experience
Are you a Speech-Language Pathologist who wants to make a real difference in patients' lives? Summit West Care, a trusted non-profit home health agency in Western Colorado, is seeking a Part-Time or PRN SLP to provide individualized, evidence-based therapy in the comfort of patients' homes.

Position ResponsibilitiesAs a key member of our interdisciplinary home health team, you will:
Evaluate and treat patients with speech, language, cognitive, voice, and swallowing disorders
Develop and implement individualized, goal-driven treatment plans
Educate patients and caregivers on techniques that enhance communication and daily function
Collaborate with nurses, PTs, OTs, social workers, and caregivers to provide coordinated care
Document care accurately in our electronic health record (HCHB training provided)
Travel within Mesa County for patient visits (mileage and drive time reimbursed)
Flexible scheduling based on your needs (Part-Time or PRN)
Competitive hourly compensation based on experience and visit type
Mileage and drive time reimbursement for every visit

Active Colorado Speech-Language Pathology license (required)
Certificate of Clinical Competence (CCC-SLP) preferred, not required
Experience in home health, rehabilitation, or medical settings preferred; new graduates considered
Strong clinical reasoning, communication, and documentation skills
Reliable personal transportation, valid driver's license, and auto insurance
