Here, you'll deliver 1:1 in-home therapy, manage your schedule, and focus on what matters most:
Helping people live safely, independently, and with dignity - at home.

• Provide individualized OT services in patients' homes

• Develop functional, patient-centered treatment plans

• Complete home safety assessments and recommend adaptive equipment

• Supervise and collaborate with a qualified COTA, as applicable

• Partner with an interdisciplinary care team (RN, PT, caregivers, families)

• Complete documentation efficiently with full EMR training and support

• Travel locally within Mesa County (mileage & drive time reimbursed)

• Active Colorado Occupational Therapist license (required)

• Home health experience preferred (new to home health? We'll train you)

• Strong clinical judgment and independent practice skills

• Ability to collaborate and provide clinical guidance to a COTA

• Excellent communication and time-management skills

• Reliable vehicle, valid driver's license, and auto insurance

• A genuine passion for helping people maintain independence

• $48–$55/hour DOE
