Summit West Care is a mission-driven, non-profit home health agency serving Mesa County and surrounding communities. We're looking for a Physical Therapist Assistant (PTA) who enjoys working independently, values flexibility, and wants to focus on patient care-without the expectation of managing a regular, productivity-heavy caseload.

This role supports our Physical Therapists by providing treatment visits as needed, helping ensure continuity of care while maintaining balance and autonomy.

• Provide skilled PTA treatment visits in patients' homes under the direction and supervision of a licensed Physical Therapist

• Carry out established treatment plans, including therapeutic exercises, gait training, and functional mobility activities

• Monitor patient progress and communicate updates to the supervising PT

• Complete timely, accurate clinical documentation

• Collaborate with the care team while working independently in the field

• Current Colorado PTA license in good standing

• Graduate of an accredited Physical Therapist Assistant program

• Valid driver's license, reliable transport, and ability to travel locally

• Comfortable working independently in home settings

• Strong communication and documentation skills
