Physical Therapist
Location: Grand Junction, CO Agency: United Health Care Staffing Pay: $60 per hour Shift Information: Days Contract Duration: 13 Days Start Date: ASAP
About The Position
Roles & Responsibilities- Perform comprehensive in-home evaluations, Start of Care (SOC), reassessments, recertifications, and discharge visits with clinical confidence
- Develop and implement individualized, evidence-based plans of care that improve mobility and independence
- Deliver skilled interventions including gait, balance, therapeutic exercise, mobility training, and fall prevention
- Educate patients and caregivers on home exercise programs and safety strategies to support lasting outcomes
- Document all care accurately and timely in Homecare Homebase (HCHB) in compliance with Medicare and agency standards
- Communicate changes in patient status to the interdisciplinary team and participate in coordinated care planning
- Provide clinical oversight and collaborate with PTAs when applicable, following Colorado law and agency policy
- Current, unrestricted Colorado Physical Therapist license
- Graduation from an accredited Physical Therapy program
- Valid driver’s license and reliable transportation for in-home visits
- Ability to practice independently in a home-based setting and make sound clinical decisions
- Proficiency or willingness to learn Homecare Homebase (HCHB)
- Comfort with Medicare documentation and home health regulations (preferred)
- Experience with geriatric or medically complex populations and supervising PTAs (preferred)
- Graduation from an accredited Physical Therapy program
- Strong patient assessment and evaluation skills in the home environment
- Expertise in plan-of-care development and skilled therapeutic interventions
- Gait training, balance training, therapeutic exercise, mobility training, and fall prevention techniques
- Accurate clinical documentation and familiarity with Medicare Conditions of Participation
- HIPAA-compliant communication and care coordination within an interdisciplinary team
- Ability to manage caseloads, prioritize visits, and work independently in the community
