• Current, unrestricted Colorado Physical Therapist license
• Graduation from an accredited Physical Therapy program
• Valid driver’s license and reliable transportation
• Ability to practice independently in a home-based setting
• Proficiency or willingness to learn Homecare Homebase (HCHB)

• Home health experience
• Familiarity with Medicare documentation and home health regulations
• Experience working with geriatric and medically complex populations

• Patient-centered, in-home care
• One-on-one visits
• Supportive clinical and administrative coordination
• Mission-driven home health services in the Grand Junction community

Monday - Friday 8 am-5 pm
