Summit West Care is seeking a compassionate Occupational Therapist to provide 1:1, in-home therapy services across the Grand Junction area. • Provide individualized occupational therapy services in patients’ homes
• Evaluate functional abilities and develop patient-centered treatment plans
• Address ADLs, safety, mobility, and independence within the home environment
• Collaborate with physicians, caregivers, and interdisciplinary clinical teams
• Support patients in achieving meaningful, functional outcomes

• Graduate of an accredited Occupational Therapy program
• Active Colorado Occupational Therapist license in good standing
• Home health experience preferred (but not required)
• Strong communication skills and ability to work independently
