The Admissions Coordinator is responsible for coordinating all aspects of the admissions process for the Skilled Nursing Facility. This full-time or part-time role manages referrals, conducts clinical screening evaluations, verifies financial eligibility, and serves as a liaison between hospitals, physicians, residents, families, and insurance providers. The Admissions Coordinator plays a key role in census development while ensuring compliance with federal and state regulations.

What You’ll Do
- Receive, document, and evaluate all new resident referrals from hospitals and community sources.
- Conduct screening assessments to determine eligibility for skilled nursing admission and appropriate level of care.
- Verify payer source and determine Medicare, Medicaid, and managed care eligibility, including insurance authorizations when required.
- Coordinate communication between referring and receiving physicians, hospital discharge planners, residents, and families.
- Schedule and coordinate pre-admission conferences and ensure required admission documentation and signatures are obtained.
- Maintain accurate documentation of admissions, discharge activity, and referral outcomes to support census tracking.
- Provide facility tours and education to prospective residents and families regarding services offered.
- Develop and maintain relationships with hospital and community referral partners to support census growth.
- Ensure compliance with federal and state regulations related to admissions, discharge, and transfer processes.
Qualifications
- High school diploma or equivalent required.
- Minimum one (1) year of healthcare experience required; long-term care or skilled nursing experience preferred.
- Working knowledge of Medicare, Medicaid, managed care, and insurance authorization processes.
- Familiarity with medical terminology and the ability to review clinical referral documentation.
- Strong interpersonal and presentation skills with the ability to communicate effectively with healthcare professionals, residents, families, and community partners.
- Ability to manage multiple referrals simultaneously, work under pressure, and make sound eligibility determinations.
- Valid State Driver’s License required.
