Summit West Care is seeking a compassionate Speech-Language Pathologist (SLP) to provide one-on-one care throughout Mesa County on an as-needed basis.

While referrals may vary throughout the year, your expertise is invaluable when patients require speech therapy services. Responsibilities
  • Perform speech-language pathology evaluations, reassessments, recertifications, and discharges
  • Assess and treat speech, language, cognitive-communication, and swallowing disorders
  • Develop and implement individualized treatment plans based on patient goals and clinical needs
  • Educate patients, caregivers, and family members on treatment recommendations and home exercises
  • Collaborate with physicians, nurses, therapists, caregivers, and other members of the care team
  • Monitor patient progress and adjust treatment plans as appropriate
  • Recommend compensatory strategies, adaptive techniques, and safety interventions when needed
  • Complete accurate and timely clinical documentation
  • Participate in interdisciplinary care planning to support optimal patient outcomes
Qualifications
  • Current Colorado Speech-Language Pathologist (SLP) license in good standing
  • Master's Degree from an accredited Speech-Language Pathology program
  • Certificate of Clinical Competence (CCC-SLP) preferred
  • Strong clinical assessment, communication, and documentation skills
  • Experience with adult and geriatric populations preferred
  • Ability to work independently in a home health environment
  • Excellent time-management and organizational skills
  • Valid driver's license, reliable transportation, and auto insurance
Home health experience is a bonus, but not required! PRN Position Information

This position is referral-based and patient needs may fluctuate throughout the year. At times there may be consistent visits available, while other periods may involve little or no active caseload. We are seeking a clinician who values flexibility and is interested in partnering with Summit West Care when speech therapy services are needed.
