Full-Time Registered Nurse (RN) – Home Health

$35–$43/hour (DOE)
Monday–Friday | 8:00 AM–5:00 PM + rotating on-call

Summit West Care is a local, non-profit home health agency serving Grand Junction and Mesa County. Our nurses deliver skilled, compassionate care in patients' homes-where healing feels personal, not rushed.

This role is ideal for RNs who want meaningful patient relationships, a consistent weekday schedule, and the support of a collaborative, mission-driven team.

• Provide skilled nursing care to patients in their homes
• Complete comprehensive assessments and develop individualized plans of care
• Monitor patient progress and adjust care as needed
• Educate patients and families on medications, treatments, and self-care
• Collaborate with an interdisciplinary care team (RNs, LPNs, therapists, caregivers)
• Maintain accurate and timely documentation using Home Care Home Base (HCHB)
• Participate in a rotating on-call schedule

• Competitive pay: $35–$43/hour (DOE)

• Active Colorado RN license (required)
• Home health experience preferred (training available for the right candidate)
• Strong clinical judgment and assessment skills
• Clear, professional communication and documentation abilities
• Reliable vehicle, valid driver s license, and current auto insurance
• A compassionate, patient-centered approach to care
