RN Case Manager

Position Highlights

• Coordinates the continuity of patient care with assigned patients and families following hospital admission, discharge, and ER visits.
• Supports patient self-management of disease and behavior modification interventions.
• Provide triage to joint and spine patients, following nursing guidelines, protocols, and physician direction.
• Ensures Western Orthopedics & Sports Medicine and Grand Valley Spine meet the standards of The Joint Commission (TJC).
• Directs preparations and ongoing monitoring of accreditation standards.
• Maintains and monitors quality matrix and files in compliance with TJC.
• Incorporate factors related to safety, effectiveness, cost, and impact on practice in planning and delivering patient care in regard to resource utilization.
• Maintains data collection, health outcomes reporting, clinical audits, and programmatic evaluation related to the Center of Excellence for Orthopedic and Spine patients as directed by The Joint Commission.
• Evaluates clinical care, utilization of resources, and development of new clinical tools, forms and procedures related to The Center of Excellence.

• Bachelor's degree in nursing
• At least three (3) years of related experience in case management and/or utilization management, or similar experience required.
• Experience in orthopedics or spine preferred.
• Certification in Case Management preferred or willing to obtain within three (3) years of employment
• Registered Nurse (RN) in the State of Colorado.

• $39.00 - $44.85 per hour, depending on education and experience.
• Bonus: $5,000
