Job Description:

The CV Nurse Care Coordinator works collaboratively with physicians and other members of the health care team to improve the health of patients with chronic conditions or complex needs. This position educates patients and families to help them manage their health care needs. The incumbent facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.

Position Details :
Monday - Thursday; 7am - 4:15pm;
Friday 7am - 12 noon Essential Functions Care Coordination:
Manages the entire patient journey, including ordering and following up on diagnostic tests, coordinating surgical scheduling, and post-procedure follow-up. Round daily on pre- and post-operative hospital patients to ensure adherence to care pathways.

Patient Education & Support:
Provide pre- and post-operative education within the clinic, address questions, and offer emotional support for patients and families.

Clinical Collaboration and

Education:

Partner with cardiothoracic surgeons, cardiologists, and nursing staff to implement and maintain evidence-based care standards. Collaborate with the nurse education team for specialized training of ICU and ward staff, and serve as the lead educator for ancillary clinic staff.

Advocacy & Problem Solving:
Identify and assist patients in overcoming barriers to care, such as transportation or social issues.

Follow-Up Care:
Conduct or schedule follow-up visits within two weeks of discharge. Serve as the primary point of contact for discharged patients regarding surgery-related concerns.

Quality Metrics:
Assist in data collection to optimize clinical care and reduce ER visits and readmission rates.

Minimum Qualifications Current license to practice as a Registered Nurse (RN) in state of practice.

Basic Life Support (BLS) Certification for Healthcare Providers Bachelor’s degree in nursing (BSN) from an accredited institution (degree will be verified). Registered Nurses (RNs) hired or promoted into this role must obtain their Bachelor of Science in Nursing (BSN) within four (4) years of hire or promotion date. If there is an existing education agreement, that agreement will take precedence.

Preferred Qualifications

Experience:

Requires 3-5 years of experience in cardiac surgery or critical care.

Skills:

Strong communication, organizational, and critical thinking skills.

Certification:
Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS)
Ongoing need for employee to see and read information, labels, assess patient needs, operate monitors, identify equipment and supplies.

- and
-Frequent interactions with patient care providers, patients, and visitors that require employee to verbally communicate as well as hear and understand spoken information, alarms, needs, and issues quickly and accurately, particularly during emergency situations.

- and
-Manual dexterity of hands and fingers to manipulate complex and delicate equipment with precision and accuracy. This includes frequent computer use and typing for documenting patient care, accessing needed information, medication preparation, etc.

- and
-Expected to lift and utilize full range of movement to transfer patients. Will also bend to retrieve, lift, and carry supplies and equipment. Typically includes items of varying weights, up to and including heavy items.

- and
-Need to walk and assist with transporting/ambulating patients and obtaining and distributing supplies and equipment. This includes pushing/pulling gurneys and portable equipment, including heavy items. Often required to navigate crowded and busy rooms (full of equipment, power cords on the floor, etc).

-May be expected to stand in a stationary position for an extended period of time.
