4‑day workweek (4x10 shifts). Average patient volume 15-17 PPD. Alternate schedule options available.

- NO CALL – Outpatient care only
- Integration with Behavioral Health providers on each clinic pod
- Robust team‑based clinic team including MAs, RNs, patient navigators, case managers, and care coordinators
- On‑site pharmacy, dental services, psychiatry services
- Active quality program with outstanding results
- Broad scope of practice including wellness visits, chronic disease management, and opportunities for in‑office procedures based on your interests
- Collaborative practice including APPs and physicians with weekly Lunch & Learns

QualificationsRequired Education/
Experience:
- Master’s degree, equivalent education, or higher degree from an accredited APN/NP program required.
- Holds and maintains without restrictions a Colorado license to practice as an APN/NP, DEA license to prescribe medication, Life Support certification (BLS at minimum).
- Maintenance of Continuing Education as required by your APN/NP credentialing board and state medical board.
Preferred Education/
Experience:
- Two years of related experience as an APN/NP.
- Unrestricted Colorado license and DEA license.
- Enjoys caring for patients of all ages.
- Experienced in integrated team‑based care, with past FQHC experience a plus.
- Bilingual a plus.
