As a Primary Registered Nurse specializing in Case Management, you will lead an interdisciplinary team to deliver exceptional care to patients in various settings, including private homes, nursing homes, group homes, and assisted living facilities.

Your role will involve:

• Managing an assigned caseload of patients
• Providing compassionate care and symptom management
• Educating patients and caregivers on care goals and strategies
• Exercising strong clinical skills and critical thinking
• Collaborating with a team to ensure comprehensive care
• Working a weekday schedule of Mon-Fri 8am-5pm, with occasional weekend and on-call duties
• Need 1 year experience in setting
