Travel RN - Case Management​/Utilization Review - Case Management

American Traveler is seeking an experienced RN Case Manager for a community hospice travel position requiring a minimum of 2 years of experience and an active CO or compact RN license.

Job Details

• Community hospice setting, providing care wherever patients call home - including private residences, nursing homes, group homes, and assisted living facilities,

• Assigned caseload of patients as the primary RN, leading an interdisciplinary care team,

• Monday-Friday, 8:00 AM-5:00 PM schedule,

• Occasional weekend backup and on-call responsibilities,

• 12-week contract,

Job Requirements

• Active CO or compact RN license currently in hand,

• Minimum 2 years of RN experience required,

• Strong clinical assessment and critical thinking skills,

• Experience in hospice, palliative care, or case management preferred,

Additional Information

• Primary responsibilities include symptom management, patient goal-of-care planning, and educating and supporting family caregivers,

• Functions as a generalist across disease types with a specialty focus on end-of-life symptom management,

• Serves as the lead RN within an interdisciplinary care team for an assigned patient caseload,
