The CV Nurse Care Coordinator collaborates with physicians and the healthcare team to improve the health of patients with chronic conditions or complex needs. This role educates patients and families to manage their healthcare needs, facilitates communication, coordinates services, addresses barriers, and promotes optimal allocation of resources while balancing clinical quality and cost management.
Position Details- Schedule:
Monday – Thursday, 7:00 am – 4:15 pm;
Friday, 7:00 am – 12:00 noon - Hours:
40 per week
- Manage the entire patient journey: order and follow up on diagnostic tests, coordinate surgical scheduling, and provide post-procedure follow-up.
- Round daily on pre- and post-operative hospital patients to ensure adherence to care pathways.
- Provide pre- and post-operative education within the clinic, address questions, and offer emotional support for patients and families.
- Collaborate with cardiothoracic surgeons, cardiologists, nursing staff, and the nurse education team to implement evidence-based care standards and serve as the lead educator for ancillary clinic staff.
- Advocate for patients and solve problems related to barriers to care, such as transportation or social issues.
- Conduct or schedule follow-up visits within two weeks of discharge and serve as the primary contact for discharged patients regarding surgery-related concerns.
- Current license to practice as a Registered Nurse (RN) in the state of practice.
- Basic Life Support (BLS) Certification for Healthcare Providers.
- Bachelor’s degree in nursing (BSN) from an accredited institution (verification will be performed). RNs hired or promoted into this role must obtain their BSN within four (4) years of hire or promotion date unless an existing education agreement applies.
- 3–5 years of experience in cardiac surgery or critical care.
- Strong communication, organizational, and critical thinking skills.
- Certifications:
Basic Life Support (BLS), Advanced Cardiac Life Support (ACLS). - Physical ability to: lift and move patients; bend, lift, and carry supplies (up to heavy items); walk and assist with transporting/ambulating patients; stand for extended periods.
