Responsibilities
  • Provide after‑hours hospice nursing care across diverse settings, including homes, assisted living, skilled facilities, hospitals, and a hospice care center
  • Quickly assess patient and family needs; determine symptom management and urgent visit interventions
  • Administer medications, monitor vitals, manage comfort measures, and respond to acute changes in patient condition
  • Develop and modify care plans in collaboration with on‑call teams; document findings clearly and promptly
  • Communicate with patients, families, and interdisciplinary team members to ensure aligned, compassionate care
  • Offer end‑of‑life support and symptom relief during after‑hours encounters; maintain safety and infection control standards
  • Work from home between patient visits and travel to patient locations as required
Requirements
  • Current, active Registered Nurse license in Colorado
  • Hospice or palliative care experience preferred; not required
  • Ability to work 7:00 pm–7:00 am shifts with rotating nights
  • Comfort delivering care across home, assisted living, skilled facilities, hospitals, and hospice settings; capable of coordinating care remotely between visits
  • Willingness to travel within the Grand Junction/Mesa County area
