Position Details
Licensed Practical Nurse (LPN)
Mantey Heights Rehabilitation & Care Center
Grand Junction, Colorado
Pay Range: $26.00 - $38.52 per hour DOE
Sign‑On Bonus: $10,000 for full‑time hires
ScheduleFull‑time schedule:
Sunday, Monday & Tuesday, 6:00 AM – 6:30 PM
PRN schedule:
Flexible scheduling available based on facility needs
- Must work a minimum of 2 shifts per month
- At least 1 shift per month must be worked on a weekend
- Must maintain active employment status and complete all required trainings and competencies
- Provide direct nursing care to residents in accordance with physician orders and individualized care plans
- Administer medications and treatments safely and accurately
- Monitor resident conditions and report changes to physicians and nursing leadership
- Complete assessments and documentation in accordance with facility policies and state and federal regulations
- Supervise and support CNAs in delivering exceptional resident care
- Collaborate with physicians, therapists, and interdisciplinary team members
- Promote resident dignity, independence, and quality of life
- Current Colorado LPN License
- Graduate of an accredited practical nursing program
- Strong clinical, organizational, and communication skills
- Long‑term care or skilled nursing experience preferred but not required
- Current CPR certification or ability to obtain upon hire
