Licensed Practical Nurse (LPN) – Long-Term Care – Contract
Facility: Eagle Ridge Post Acute
Facility Type: LTC / Nursing Home
Location: Grand Junction, CO
Duration: 13 Weeks
Shift: Nights (Variable)
Hours: 12-Hour Shifts
Guaranteed
Hours:
36 Hours/Week (Thursday–Saturday)
Pay Rate: $40 – $43/hr
Estimated Weekly Pay: $1,440 – $1,548/week
Eagle Ridge Post Acute is seeking a dependable and experienced LPN to provide compassionate nursing care for residents in a long‑term care and skilled nursing environment. The LPN will administer medications, monitor resident conditions, document care, and collaborate with the interdisciplinary team to ensure quality patient outcomes.
Requirements- Active Colorado LPN License
- Graduate of an accredited Practical Nursing program
- Skilled Nursing Facility (SNF) and/or Long‑Term Care experience required
- Strong medication administration and patient assessment skills
- Reliable attendance and ability to work independently
- Willingness to pick up overtime shifts as needed
- Must be willing to work weekends when required
- Active Colorado LPN License
- Diploma or Associate Degree in Practical Nursing from an accredited program
- BLS Certification (preferred or facility required)
- Long‑Term Care (LTC)
- Skilled Nursing Facility (SNF)
- Post‑Acute Rehabilitation
