Registered Nurse (RN) – Home Health
Location: Grand Junction, CO
Duration: 13 Weeks
Pay Rate: As Expected.
We are seeking a Registered Nurse (RN) for Home Health at Hope West Hospice. This is a 13-week contract with competitive pay and the opportunity to provide compassionate, community-based care to elderly participants through the PACE (Program of All-Inclusive Care for the Elderly) program.
Specialties / UnitsHome Health RN
The ideal candidate is experienced in acute care and comfortable delivering high-quality nursing care in a home health setting while working independently and coordinating with an interdisciplinary team.
Requirements- Active RN License (CO or Compact)
- Minimum 2 years of recent hospital or acute care experience
- Home Health And/or LTC Experience Preferred
- Current CPR certification
- Valid driver’s license, reliable transportation, and auto insurance
- Strong communication and organizational skills
- Experience with EHR systems and Microsoft Office
