Candidate Pay Rate: $56 per hour

• Provide compassionate, life-changing care to patients and families in their homes, nursing homes, or assisted living facilities
• Lead the interdisciplinary team managing an assigned caseload of hospice patients
• Teach patients to identify personal goals of care and empower caregivers with confidence
• Excel in symptom management and assessment across all disease types
• Collaborate with a supportive team that values personalized patient care

• Active RN license in Colorado (or compact state) required
• Minimum 2 years of nursing experience
• Hospice or palliative care experience strongly preferred
• Strong clinical assessment and critical thinking skills
• Passion for teaching and supporting patients and families

• Strong clinical assessment and symptom management
• Teaching and communication skills
• Interdisciplinary teamwork and leadership
• Adaptability to various home environments
