Position: Travel Registered Nurse (RN) Case Manager-44

RN Case Manager

We are seeking a compassionate and dedicated Registered Nurse (RN) Case Manager to join our healthcare team. The RN Case Manager will be responsible for coordinating the care of patients throughout their healthcare journey, ensuring quality, continuity, and efficiency in their treatment. This role requires strong clinical skills, critical thinking abilities, and excellent communication with patients, families, and healthcare providers.

Responsibilities
  • Develop and implement personalized care plans for patients in collaboration with physicians and other healthcare professionals.
  • Monitor and evaluate patient progress and make adjustments to the care plan as needed.
  • Coordinate healthcare services, such as medications, treatments, and appointments, to ensure optimal outcomes for patients.
  • Provide education and support to patients and their families to promote self-management of their health conditions.
  • Advocate for patients to ensure access to necessary resources and services.
  • Maintain accurate and up-to-date patient records and documentation.
  • Communicate effectively with healthcare team members to facilitate coordinated care.
  • Participate in case conferences and interdisciplinary meetings to discuss patient care and treatment options.
  • Stay current on healthcare policies, regulations, and best practices in case management.
Qualifications
  • Active Registered Nurse (RN) license in the state of practice.
  • Bachelor's degree in Nursing (BSN) preferred.
  • Minimum of 2 years of nursing experience, with previous case management experience preferred.
  • Strong clinical assessment and critical thinking skills.
  • Excellent communication and interpersonal skills.
  • Ability to work independently and as part of a healthcare team.
  • Knowledge of healthcare policies, regulations, and best practices in case management.
  • Proficiency in electronic health record (EHR) systems.
